Credentialing is the process of being recognized by managed healthcare organizations (MHO's) as a licensed and qualified provider. The process can also involve joining an HMO/PPO network as a preferred provider which may increases patient volume at the cost of accepting a reduce reimbursement amount. MHOs are either government (Veterans, Tricare, Prisons, Medicare, Medicaid) or private (HMOs, PPOs, Indemnity Insurance).

The process typically involves a lengthy application form and submission of source documents such as certificates, proof of malpractice insurance, history of lawsuit claims paid or settled, school transcripts, etc. Some states have one standard form that is accepted by several MHOs.
